Note: The Candidates are requested to bring All Original Certificates including Registration Certificate of Assam Nursing, Midwives’ and Health Visitors’ Council along with two sets of Self-Attested photocopies of all relevant testimonials and two (02) passport size photographs. All documents will be verified during the process of counseling.
Documents required to be brought on the day of counseling by the candidates:
(a) Original mark sheets & certificates as per the eligibility criteria as mentioned in the advertisement.
(b) Self Attested copies of mark sheet and certificate of relevant qualification as per the eligibility criteria as mentioned in the advertisement.
(c) Self Attested copies of matriculation certificate/admit card/pass certificate in support of candidate’s age
(d) Registration of State Nursing Council, Assam
(e) Copy of Caste Certificate(if the candidate belongs to SC/ST/OBC category)
(f) Two (02) recent passport size photographs
(g) Copy of earlier Appointment Letter issued by NHM, Assam, if already serving under NHM.
